Position Description

Licensed Practical Nurse (LPN/LVN)

Department: Nursing

Reports to: Director of Nursing

Prepared by: Human Resources

As an LPN/LVN in our long-term care facility, you’ll be a key contributor to resident health and well-being. You’ll deliver hands-on nursing care, supervise nursing assistants, and help maintain a safe, compassionate, and compliant environment. This is a role where your clinical skills, leadership, and empathy truly make a difference.

Core Responsibilities

Resident Care & Clinical Duties

  • Provide direct nursing care, including medication administration, treatments, and specimen collection.
  • Monitor resident conditions, respond to changes, and communicate with physicians and families.
  • Support restorative and rehabilitative programs and ensure care aligns with resident plans and preferences.

Team Supervision & Coordination

  • Lead and guide nursing assistants during your shift.
  • Assign tasks, monitor performance, and ensure compliance with care standards.
  • Participate in staff evaluations and support training and development.

Documentation & Compliance

  • Maintain accurate and timely charting, including admissions, transfers, discharges, and incident reports.
  • Transcribe physician orders and ensure proper medication handling and documentation.
  • Uphold HIPAA and facility data security protocols.

Safety & Infection Control

  • Enforce safety standards and hygiene practices.
  • Ensure proper use of PPE and participate in fire/disaster drills.
  • Support infection control procedures and report hazards or exposures.

Care Planning & Communication

  • Review and update resident care plans.
  • Ensure CNAs are informed and follow care plans.
  • Collaborate with interdisciplinary teams to ensure continuity of care.

Qualifications

✅ Education & Experience

  • Graduate of an accredited LPN/LVN program or nursing degree.
  • Active, unencumbered LPN/LVN license in the state.
  • CPR certification required.
  • Experience with PointClickCare (PCC) preferred.
