Key Responsibilities
- Administer skilled nursing care under RN supervision.
- Perform curative, rehabilitative, or preventive care tasks as outlined in patient care plans.
- Utilize clinical expertise and nursing education to make informed, independent decisions that support patient care and promote positive outcomes.
- Complete visit reports promptly and accurately within the electronic medical record system to support high-quality care and regulatory compliance, including all required OASIS documentation.
- Confirm daily visit schedules to ensure seamless coordination with other staff.
- Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
- Understand and adhere to the agency’s process for managing and addressing customer complaints.
- Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.
- Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.
- Must have a current LPN or LVN license (state-specific).
- Graduate of an accredited nursing program.
- Valid BLS certification.
- Minimum one year of nursing experience (home health or medical/surgical preferred).
- Excellent communication, organizational, and time management skills.
- Must have a valid driver’s license and access to a reliable and insured vehicle.
- The ability to visit homes that may not be wheelchair accessible.
- Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
- Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
