TITLE: Clinic Nurse

JOB OVERVIEW: The Clinic Nurse plays a vital role in delivering high-quality patient care within an outpatient clinic setting. This position will work collaboratively with clinical care teams in the coordination of care. Receives and responds to calls and inquiries from patients and families for health care assistance, using the nursing process in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization.

DEPARTMENT: Clinic Network

WORK HOURS: Monday- Friday; Hours may vary to meet departmental and patient needs.

REPORTS TO: Clinic Manager

Prerequisites:

Graduation from an accredited school of nursing approved by the Washinton State Board of Nursing.

BSN preferred.

Current Washington Registered Nurse (RN) License required.

AHA BLS for healthcare providers required.

Minimum 3 Years' Experience As a Registered Nurse, Required.

Preferred hospital-based environment experience in specialty area.

Requires sitting for extended periods of time.

Essential Responsibilities And Competencies:

Use Nursing Process to assess, plan, implement, and evaluate the needs of the patient

Performs and delegates a variety of tasks to facilitate patient care.

Serves as a clinical resource and provides guidance to call center staff for clinical questions as appropriate.

Seeks opportunities to improve or enhance triage and patient advice services within the clinic.

Evaluate and respond appropriately to urgent or emergent situations.

Triage and respond to incoming patient calls and MyChart messages that are beyond the scope of the MA and within the RN scope of practice, with the goal of first calls resolve.

Support medication refills per established protocols.

Provide the clinic pre-visit assessment as needed.

Provide appropriate education and instructions to patients and family regarding treatment plans.

Provide initial patient education and follow-up reinforcement at scheduled patient appointments.

Monitor device/s and consult licensed providers as indicated to support optimal patient care.

Coordinate patient referrals, appointments, testing and admissions to other facilities.

Contact patients to relay test results and care instructions, according to ordering physician/APPs.

Provide oversight, direction, and assist, as necessary, with in-clinic medications and procedures.

Perform Medicare Annual Wellness Visits.

Meet expectations in established service line metrics.

Manage patient populations requiring care management support by assessing healthcare and psychosocial needs, identifying resources needed, ongoing educational needs, management of chronic condition and access to community resources, as well as a partnership with the primary provider of care.

Performs and delegates a variety of tasks to facilitate patient care.

Utilizes decision support tools for symptom based and emergent care.

Assure timeliness and quality of documentation and billing compliance.

Performs other duties as assigned.
