What We Offer

Schedule: PRN; Shifts may be available any day of the week between the hours of 8:00 AM and 5:00 PM; must be able to train during the week for several weeks

Location: Presbyterian Medical Center - Charlotte, NC; Presbyterian is our 661 bed level 2 trauma center centrally located just outside of uptown Charlotte; will be eligible to float to other hospitals in the region

This role is ideal for an experienced acute care RN looking to work in a patient facing but non-bedside role within a high-performing, collaborative team.

What You'll Do

  • Complex discharge planning
  • Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive/complex/safe discharge plans
  • Collaborate with MSW case managers for guidance with pscyhosocially complex cases
  • Facilitate coordination of care
  • Accept responsibility, authority, and accountability for management of care
  • Serve as a resource for discharge planning and care coordination for medically complex cases
  • Maintain current knowledge of available community resources/post acute care options

What We're Looking For

  • Education: Associates Degree in Nursing or BSN, required. 4 Year / Bachelors Degree, preferred.
  • Experience: Two or more years healthcare experience, preferred. Case Management or related experience, preferred.
  • Licensure/Certification: Current RN licensure in state of employment, required.
  • Additional skills required: Weekend and holiday coverage.
  • Additional skills preferred: Knowledge/Competence of Microsoft office products and keyboarding.
