TITLE: Oncology Nurse Navigator
JOB OVERVIEW: Guides patients through the health care system by assisting with access issues, developing relationships with service providers and tracking interventions and outcome. Promotes a positive teamwork environment which emphasizes respect to all customers, including patients, families, physicians, staff, and other departments. Receives and responds to calls from patients and families for health care assistance using the nursing process in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization. This position will also work collaboratively with clinical care teams in the coordination of care for certain patient symptoms and conditions.

WORK HOURS: Monday - Friday; hours vary by posting
REPORTS TO: Manager, Medical Oncology Clinic
Prerequisites
Graduate of an accredited nursing program. Bachelor of Nursing degree, preferred.
Current Washington Registered Nurse (RN) License.
Minimum two (2) years nursing experience in Oncology OR five (5) years nursing experience, required.
Nursing Experience In Outpatient Clinic Setting, Preferred.
ONCC certification preferred.
Qualifications
Proficient in using Microsoft Office suite.
Experience Working With Electronic Health Record, EPIC Preferred.
Demonstrated knowledge and use of the nursing process to identify physical, psychosocial, and developmental needs and interventions.
Excellent oral and written communication skills working in diverse setting
Demonstrated commitment to patient centered care.
Demonstrated critical thinking skills, problem solving and excellent clinical judgment.
Demonstrated flexibility and ability to prioritize and manage multiple demands.
Able to work collaboratively with a wide range of disciplines on the health care team.

Essential Responsibilities And Competencies
Facilitates the entry of patients into the diagnostic and/or treatment system and coordinates their care including family involvement. Follows the patients throughout the continuum of health care services across multiple departments acting within their scope of practice.
Implements risk assessments and healthcare pathways for cancer patients, based upon the clinical staging of the patient. Evaluates progress of patients along the continuum and is responsible for professional and personal modifications as directed by the healthcare team as appropriate.
Develops relationships with all oncology providers including but not limited to medical oncologists, radiologist, general surgeon, surgical oncologist, radiation oncologist, pathologist, plastic surgeon, primary care physician, hospital departments and team members. Serves as a liaison between the patient, hospital departments, team members and the providers.
Comprehensively assess the patient's physical and psychological status.
Provides approved patient education across the continuum of care and provides symptom management. Provides test results, as directed by physicians.
Provides access to community resources and links patients to support services such as: social services, nutrition services, psychologist, pastoral care, physical therapy, rehabilitation therapy, financial counseling, etc.
Assists with data management and entry for tracking quality metrics for diagnosing and treating all cancers promptly and effectively.
Develop and implement approved workflow processes for the center that will serve in delivering high quality care to patients within the community.
Participate in the survivorship program.
Coordinate site specific cancer patient multidisciplinary (MDC) visit, document treatment plan summary and disseminate to patient, appropriate departments, and physicians.
Participate in site specific cancer multidisciplinary tumor conference; document recommendations and monitor for appropriateness with NCCN or other national guidelines.
Participate in the national accreditation process for the cancer center program.
Develop in conjunction with support staff, cancer support groups and education programs for the patient, their families and/ or caregivers.
Participate in the Patient Advisory Committee and /or Patient and Family Centered Care Committee.
Participation in process improvement work that supports organization of quality and safety initiatives.
As a member of a health care team, functions in the role of patient advocate by being in the position of initial contact for patient-related issues.
Provides support and coverage for clinic care management team.
Additional duties as assigned
Telephone Assessment, Triage, And Care Management
Manages patient health concerns and symptoms via a telephone interaction utilizing approved protocols for the assessment and management of a range of symptoms in the Primary Care and/or Specialty Care setting.
Performs in-patient assessment, care coordination and professional nursing functions to assess the severity of the patient's health concerns using procedures, protocols and algorithms in a manner that provides patient dispositions to emergency care or clinic provider.
Gathers and assesses objective and subjective data from the patient.
Executing medical regimens and nursing interventions as appropriate for the setting and their competency.
When appropriate, contacts appropriate emergency response services.
Provides referrals to healthcare providers and community resources when appropriate, including locations, hours, and contact information.
Schedules appointments as needed.
Provides Education for patient and/or family members, according to the individualized needs of the patient and as prescribed by the Provider and/or Group policy.
Responds to patient questions and appropriately provides reassurance while responding to patient concerns.
Documentation In The Electronic Health Record
Provide and document accurate and detailed assessment in patient records.
Enters all pertinent communications with the patient into the Medical Record.
Navigates and documents in the electronic health record while simultaneously talking with the patient or family member on the phone.
