Home Health Registered Nurse – PA

Job Summary

The Home Health Registered Nurse provides skilled nursing care to patients in their homes under physician orders. Responsibilities include patient assessment, care planning, implementation of nursing interventions, and evaluation of outcomes. This role adheres to the State Nurse Practice Act and home health care standards.

Minimum Qualifications

• Experience: At least 1 year of clinical home health experience in skilled home health care using OASIS visits.
• Note: Private duty experience does not qualify.
• When substituting, candidates must confirm OASIS/skilled home care experience.
• Education: Graduate of an accredited School of Nursing (BSN preferred).
• Licensure/Certification:
• Active Registered Nurse License in the state of practice
• Valid Driver’s License
• Current CPR Certification
  

Skills & Competencies

• Strong oral, written, and interpersonal communication skills
• Knowledge of nursing principles, practices, and home health standards
• Ability to assess patient needs and develop individualized care plans
• Team-oriented, able to collaborate within an integrated healthcare team
• Proficiency with office equipment (tablet, cell phone)
• Ability to perform professional nursing procedures in a home care setting
• Willingness to travel within the service area
• Flexibility to work weekends, holidays, and on-call shifts
