Registered Nurse (RN) Care Manager – Make a Lasting Impact

We’re seeking a Registered Nurse Care Manager to join a dynamic care management team dedicated to supporting people with complex and chronic conditions. What You’ll Do:

• Conduct comprehensive assessments and develop tailored care plans
• Use motivational interviewing to engage and empower patients
• Coordinate care across multiple settings and providers
• Provide education, medication reconciliation, and discharge planning support
• Connect patients to primary care, behavioral health, social services, and community resources
• Collaborate closely with an interdisciplinary team to remove barriers to care

Qualifications:

• Active MA RN license (LPN with strong Care Management background considered)
• 3–5 years of nursing experience (home health, ambulatory care, community/public health, or case management ideal)
• Bilingual in Spanish strongly preferred
• Care Management certification a plus (CCM, ANCC RN-BC)
• Valid driver’s license
