Position Overview
As a Hospice RN Case Manager, you will provide compassionate, professional nursing care to hospice patients in accordance with physician orders, state regulations, and organizational policies. You will lead the care planning process, manage symptoms, and support patients and families through end-of-life care. This role is central to ensuring dignity, comfort, and quality of life for those we serve.

Position Overview
As a Hospice RN Case Manager, you will provide compassionate, professional nursing care to hospice patients in accordance with physician orders, state regulations, and organizational policies. You will lead the care planning process, manage symptoms, and support patients and families through end-of-life care. This role is central to ensuring dignity, comfort, and quality of life for those we serve.
Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized, outcome-focused care plans.
- Educate patients and families on disease progression, symptom management, and end-of-life care.
- Coordinate care across disciplines and make appropriate referrals to ensure seamless service delivery.
- Monitor and report changes in patient condition promptly, updating care plans as needed.
- Provide anticipatory guidance and emotional support to patients, families, and caregivers.
- Document all assessments, interventions, and communications accurately using our advanced EMR system.
- Serve as a resource and mentor for Hospice Aides and LPN/LVNs.
- Demonstrate cultural sensitivity and uphold organizational values in all interactions.
- Graduate of an accredited nursing program; current RN license in the state of practice.
- Minimum one year of acute care nursing experience; hospice or home health experience preferred.
- Strong communication and interpersonal skills, with an emphasis on grief and loss support.
- Valid driver’s license and reliable transportation.
