Responsibilities

• RN Case Manager – As a key coordinator of patient care, the RN Case Manager develops individualized care plans, collaborates with interdisciplinary teams, and ensures patients receive the right services for optimal health outcomes at home. This role involves direct patient visits, education, and advocacy.
• RN Visit Nurse – Focused on providing hands-on clinical care, the RN Visit Nurse completes scheduled visits to administer treatments, monitor patient progress, and provide education. This role ensures patients receive ongoing, high-quality nursing care in their homes.
  
  

Qualifications

Education:

• Graduate of State Board-approved program for Registered Nurses
  
  

Experience:

• One year of nursing experience caring for a similar patient population to be assigned or apply for our RN New Grad Residency for Home Care.
  
  

Licensure/Certification:

• Valid license as RN in state(s) where providing care.
• Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
• ANCC certification in area applicable to home care patient population COS-C
• Mandatory Reporter: Child & Dependent Adult Abuse.
• CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
