Toledo, OH | Competitive Pay $52-55/HR | Full-Time

What You’ll Do

Provide ongoing in-home assessments & nursing care
Develop & manage individualized care plans
Educate and support patients & caregivers
‍⚕️ Collaborate with healthcare providers to ensure coordinated care
Maintain accurate EMR documentation & ensure compliance
✅ Participate in quality improvement initiatives

What We’re Looking For

Current RN license in Ohio (required)
1 year acute care or med-surg experience (home health preferred)
Proficiency with EMR & Microsoft Office
Valid driver’s license & reliable transportation
