Biweekly Hours: 80.00
Shift: 7a-330p

Summary
Coordinates patient admissions, placement, and transfers to ensure a safe, efficient, and high-quality patient care environment. Collaborates with Nursing Services, Rehab admission team, administration, and other departments to support timely admissions, optimize staffing, ensure compliance with regulatory standards, and promote patient safety. Serves as a primary resource for rehabilitation admissions, providing guidance on nursing capabilities, policies, procedures, and patient authorization requirements. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.
Qualifications
Required: Graduate of an accredited school of professional nursing and current Ohio RN license. 3 years’ experience in a healthcare setting, with a focus on patient admissions, placement, or staffing coordination. Proficiency in Kronos or similar scheduling systems and IRFPAI documentation requirements. Knowledge of CMS, CARF, and Joint Commission accreditation standards. Strong communication, critical thinking, and organizational skills, with the ability to work effectively in a multidisciplinary environment. Computer literate with data entry/retrieval capability. Strong organizational skills. Excellent written, verbal, and interpersonal communication skills. Ability to learn insurance certification/re-certification requirements for long-term care. Preferred: Bachelor’s degree in nursing. 1 year of supervisory or charge nurse experience. Experience in inpatient rehabilitation, admissions coordination. Physical Requirements: May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment required. May need to lift up to twenty-five (25) pounds on occasion. Ability to communicate in face-to-face, phone, email, and other communications. Ability to see computer monitor and departmental documents.
