Compensation

Pay Range: $92,000-$94,000 Per Year

Union Position: $92K, with an additional $2K for bilingual (Spanish) candidates

Position Summary

Provide nursing care and support to program participants, including health assessments, treatment planning, and coordination of medical services.
Collaborate with the ACT team to ensure participants receive integrated physical and mental health care.

Responsibilities

Conduct nursing assessments and develop individualized care plans
Implement and evaluate nursing care
Coordinate health services with the ACT team and other providers
Lead group sessions on health, wellness, and recovery
Educate staff on medical care and medication procedures
Advocate for participants to access medical care and community supports
Supervise LPNs on medication management
Ensure participants have primary care providers and receive regular exams
Administer medications, vaccines, and perform basic procedures
Maintain accurate health records and documentation
Provide culturally competent care and crisis intervention as needed

Qualifications

Licensed and registered as a professional nurse in NYS
BSN preferred
Valid driver's license
Bilingual (Spanish) desirable
Experience with serious mental illness, forensic populations, co-occurring disorders, or HIV/AIDS preferred
