Licensed Practical Nurse (LPN)

Pay Range: $24.90 – $36.56 per hour

Schedule: Full-Time | Monday–Friday, 8:00 AM–4:30 PM | Rotating On-Call

Requirements to Apply

  • Active Pennsylvania LPN license
  • Minimum of 1 year of experience working with older adults or medically complex individuals
  • Current CPR certification
  • Valid driver’s license, reliable vehicle, and proof of auto insurance
  • Willingness to travel throughout Allegheny County

Job Overview

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our mission-driven team in Pittsburgh, Pennsylvania. In this full-time role, you’ll deliver personalized, community-based nursing care to older adults throughout Allegheny County, helping participants remain independent and safe in their homes.

Unlike traditional facility-based care, this position allows you to form meaningful, long-term relationships with participants while providing nursing services that promote dignity, health, and quality of life. You’ll work as part of an interdisciplinary care team, collaborating with nurses, social workers, and therapists to create and carry out individualized care plans that truly make a difference.

Responsibilities

  • Provide direct nursing care to participants in home and community settings
  • Support chronic disease management, medication administration, and participant education
  • Monitor participant health status and report changes to the interdisciplinary team
  • Perform assessments, wound care, and other nursing interventions as appropriate
  • Participate in team meetings and contribute to coordinated care plans
  • Maintain accurate documentation and ensure timely communication across disciplines
  • Foster trust and build ongoing relationships that enhance participant well-being

This position is based in Pittsburgh, PA, with regular travel throughout Allegheny County. Most care is delivered in participants’ homes or community spaces, offering a more personal, flexible, and rewarding nursing experience.
