Position Description
Remote Medical Case Manager (RN or LVN) – Bilingual Korean
Location: Fully Remote (Must reside in Southern CA)
Schedule: Full-Time | Monday–Friday, 8:00 AM–5:00 PM PST (some flexibility available)

Position Overview
We are seeking a compassionate and clinically strong Remote Medical Case Manager to support a high-acuity, dual-eligible member population. This role focuses on comprehensive assessments, care planning, and ongoing care coordination for members with chronic and complex medical needs, including heart disease and diabetes.
This position is fully remote, with optional opportunities to engage in in-person member events 1–2 times per quarter in Southern California for those interested.
Day-to-Day Responsibilities
- 90% computer-based work and 25%+ telephonic member interaction
- Manage a high-acuity caseload of members with multiple chronic conditions
- Collaborate closely with interdisciplinary care teams, providers, and caregivers
Key Responsibilities
1. Comprehensive Member Assessments
- Conduct thorough medical, social, and behavioral health assessments
- Identify members appropriate for case management using clinical judgment and established triggers
2. Individualized Care Planning
- Develop, implement, and manage individualized care plans in collaboration with members, caregivers, physicians, and care teams
- Continuously monitor outcomes and update care plans based on member progress and evolving needs
3. Ongoing Case Management & Outreach
- Maintain an active caseload and provide regular telephonic and virtual outreach
- Coordinate care across medical and behavioral health services
- Address barriers to care and support continuity of services
4. Member Education & Engagement
- Educate members using evidence-based guidelines and motivational interviewing techniques
- Encourage self-management, treatment adherence, and healthy behavior changes
5. Documentation & Compliance
- Accurately document assessments, care plans, interventions, and follow-up activities
- Ensure all documentation meets regulatory, quality, and timeline requirements
Required Qualifications
- Active California LVN or RN license (required)
- Fully bilingual in Korean (speaking, reading, and writing)
- Previous experience working in a remote setting
- Must be fluent in both formal and informal Korean, with cultural competence when communicating with elders
- Strong computer skills and comfort working in a remote environment
