Title: Registered Nurse ACT Team NYC - 1 day hybrid

We are seeking a compassionate and skilled Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team.

This role involves providing holistic, person-centered nursing care to individuals with serious mental illness (SMI) and co-occurring disorders in a community-based setting.

Essential Duties & Responsibilities

• Provide medication education, administration, and monitoring to ACT clients.
• Conduct comprehensive nursing assessments, focusing on both physical and mental health needs.
• Work closely with psychiatrists, therapists, case managers, and peer specialists to develop and implement individualized treatment plans.
• Support clients in managing chronic health conditions and accessing medical care.
• Offer health education on topics such as medication adherence, nutrition, diabetes management, and harm reduction.
• Participate in daily ACT team meetings and interdisciplinary care planning.
• Respond to crises and provide on-call support as needed.
• Ensure compliance with state and federal regulations, maintaining accurate clinical documentation.
  
  

Required Education And Experience

• Licensed Registered RN within the state of New York.
• Must have excellent verbal, written and interpersonal communication skills.
  
  

Preferred Qualifications & Skills

• Experience working with behavioral health and substance abuse clients ideal
• Experience working in ACT, mobile crisis, or forensic mental health settings
