We’re seeking a dedicated and compassionate Hospice RN Case Manager to provide high-quality, patient-centered care in Grand Junction, CO. Coverage Area: Travel throughout the territory required, primarily Mesa County

Schedule: Monday-Friday | 8:00 AM - 5:00 PM

As An RN Case Manager, You Will

• Assess and monitor patients’ physical, emotional, and psychological needs
• Develop hospice care plans aligned with patient wishes and goals
• Provide direct nursing care, including medications, treatments, and interventions
• Manage pain and symptoms effectively
• Educate and support patients’ families and caregivers
• Collaborate with an interdisciplinary care team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required
