RN Case Manager

StartDate: 1/12/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Discharge planning and progression; Hard conversations with family, patients and open discussion with MD for plans of care

MINIMUM REQUIRED QUALIFICATIONS – TX/compact RN license; BLS; 1+ year recent case management experience in an acute hospital setting: Meditech or HCA experience; Bed side case management experience at least 1 year; Post acute care knowledge

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – MON-FRI 8:00A-5:00P. Rotating EOW.

SYSTEMS – Meditech/Midas

START DATE – 1/12/2026
