Position Description

We are seeking a skilled and compassionate Clinical Nurse Case Manager to join our licensed home care agency (LHSCA) in Queens. This full-time onsite role offers the opportunity to lead a dedicated nursing team while ensuring the highest quality of care for our clients. The position combines office-based management with occasional field visits, providing a dynamic and impactful work environment.

Location: Queens

Schedule: Monday-Friday (no weekends!)

Salary: $100,000-$105,000 Per Year

Key Responsibilities

  • Supervise, mentor, and support nursing staff to deliver exceptional client care.
  • Oversee client care plans and ensure compliance with home care regulations.
  • Conduct occasional home visits to assess client needs and staff performance.
  • Maintain accurate medical records and documentation.
  • Collaborate with administrative and clinical teams to optimize care delivery.
  • Utilize general office software for scheduling, reporting, and communication.

Qualifications & Requirements

  • Current NY State Registered Nurse (RN) license
  • Minimum 3 years of experience in community or home care nursing
  • Prior supervisory or management experience in a clinical setting
  • Strong organizational, leadership, and communication skills
  • Valid driver’s license and reliable access to a vehicle
  • Proficiency in general office software (e.g., MS Office, electronic health records)
  • Bachelor of Science in Nursing (BSN) preferred
  • Experience enforcing home care regulations and standards
  • Bilingual in Spanish preferred
