Registered Nurse (RN) Clinical Lead - Greene County

The Community Hospice has a wonderful opportunity for a FT RN Team Leader to work as part of our Hospice Care Team caring for patients and families residing in Greene County and surrounding.

Work days are Monday-Friday 8a-4:30p

What You Will Do

• The clinical leader engages in activities that support the planning, coordination and implementation of patient care under the direction of the patient care supervisor.
• The RN also carries a small caseload which includes the implementation of an individualized plan of care including but not limited to managing discomfort and providing symptom relief.
• This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers)..
  
  

What You Will Need

• Valid NYS RN license
• Associates degree in nursing, BSN preferred
• 2 years of experience as a RN required - preferably Hospice homecare, or in an acute care environment
• Previous home care / hospice experience preferred but NOT required.
• Strong engagement to promote a strong team culture
• Valid driver license and reliable vehicle
• Understanding of Hospice principles preferred

Pay Range: $36.50-$49.95 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location
