Hospice Registered Nurse (RN) Case ManagerField-Based | Full-Time

Conduct comprehensive assessments addressing physical, psychosocial, emotional, and spiritual needs
Develop, implement, and coordinate individualized interdisciplinary plans of care
Collaborate closely with physicians, Clinical Supervisors, and the hospice care team to ensure continuity of care
Educate patients, families, and caregivers on hospice philosophy, pain management, and symptom control
Provide compassionate support at time of death and complete bereavement assessments
Communicate changes in patient condition and environmental concerns promptly and effectively
Serve as a clinical resource and educator for staff in alternate care settings such as nursing homes and assisted living communities
Hospice experience is highly preferred
Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Bachelor’s degree)
Current, unrestricted State RN license
Minimum of 1 year of clinical nursing experience
Ability to travel within the service area (approximately 50%)
Ability to lift 50–100 lbs and perform hands-on patient care
Valid driver’s license, auto insurance, and reliable transportation
