Position Summary
We are looking for a Licensed Practical Nurse to render or assign resident care according to the needs of the residents under the direction of the supervisor and or director of nursing. Maintain a home like, person centered care environment for all residents. As an interdisciplinary team member to meet the physical, psychological, and social needs of the residents within our Americare communities.
Essential Functions
The licensed nurse will serve as a liaison between the Home Health and Americare communities to provide appropriate care to our residents. The LPN is responsible for all nursing services within their scope of practice to implement all nursing objectives, standards of care, policies and procedures, and review and revise as necessary. They will determine nursing care standards are being followed according to written guidelines. They will work closely and follow in the director of our Home Health and facilities Administrators and Director of Nursing’s.

Position Summary

The LPN is responsible for all nursing services within their scope of practice to implement all nursing objectives, standards of care, policies and procedures, and review and revise as necessary. They will determine nursing care standards are being followed according to written guidelines. They will work closely and follow in the director of our Home Health and facilities Administrators and Director of Nursing’s.
- Give clinical guidance to other nursing staff (PCA, C.N.A., CMT etc.) as necessary.
- Makes rounds to assess resident and staffing needs and visits residents each work day.
- Works under the direction of director of nursing to assure that all nursing personnel is complete
- Responsible for nursing assessment on an as needed basis, documenting assessments, notifying physicians and families of change in condition, obtaining appropriate orders and implementing order.
- Responsible for seeing that all the necessary written documents such as Nursing Notes
- Communicates with facility Administrator and Director of Nursing.
- Available for call when needed by the members on duty in matters pertaining to nursing care.
The Successful Candidate Will
- Have basic knowledge over Home Health
- Detail oriented and ability to maintain accuracy.
- Ability to maintain confidentiality related to facility and residents information.
- Graduate of an approved school of nursing
- Current LPN license
- Maintain current state registration and ongoing education p. We offer employees access to wages on demand
