Position Description


Registered Nurse (RN) - Case Management / Home Health

Shift Requirements:

  • Monday-Friday, 8:00 AM-5:00 PM

  • Alternating on-call weekends (Sunday-Saturday)

Licensure Requirements:

  • Active Colorado RN license required

  • Compact (multistate) license accepted

Mileage Reimbursement:

  • $0.43 per mile

Service Area:

  • Thornton

  • Boulder

  • Broomfield

  • Golden

  • Brighton

  • Westminster

  • Arvada

  • Northglenn

Transportation Requirement:

  • Must have a reliable, working vehicle and active auto insurance

Productivity Requirement:

  • Required to complete 30 points per week

Preferred Skills & Experience
  • Homecare Homebase (HCHB) - required

  • OASIS documentation

  • Wound care and wound VAC management

  • IV therapy and PICC line management

  • Foley catheter care

Primary Responsibilities
  • Perform comprehensive assessments of high-risk members

  • Collaborate with primary care providers to develop and implement individualized, comprehensive care plans

  • Identify, address, and continuously reassess cost-effective and appropriate levels of care

  • Arrange non-medical supports (e.g., housing, transportation) to promote adherence to treatment plans

  • Coordinate care transitions across settings as needed

  • Work collaboratively with physicians, social workers, discharge planners, and claims professionals to ensure appropriate levels of care

  • Engage specialty and community resources to achieve optimal patient outcomes

  • Maintain detailed documentation of clinical, functional, and fiscal outcomes throughout the care management process

  • Identify opportunities for health promotion and illness prevention

  • Proactively prevent adverse patient events when possible and intervene promptly to minimize negative outcomes

  • Serve as a patient advocate while maintaining strict confidentiality and privacy standards

Qualifications
  • Associate's or Bachelor's degree in Nursing or related field

  • Current, unrestricted Registered Nurse (RN) license

  • Strong ability to analyze complex medical information and make sound clinical decisions

  • Case Management certification preferred
