The Home Health Registered Nurse Team Lead will oversee home health operations in our care teams, coordinating and managing a group of nurses and clinicians providing skilled nursing care to patients in their homes. Job Summary:
The Home Health Registered Nurse Team Lead will oversee home health operations in our care teams, coordinating and managing a group of nurses and clinicians providing skilled nursing care to patients in their homes. This role involves a blend of clinical expertise, leadership capabilities, comprehensive patient assessments, and effective communication skills to ensure optimal patient outcomes.

- Leadership & Supervision
- Clinical Assessment and Care Coordination
- Quality Assurance, Compliance, and Emergency Response
- Communication and Documentation
- Training and Education
Education and Experience Qualifications:
- Be a registered nurse (R.N.) with a current license in the State of practice.
- Current Basic Life Support Certification
- Minimum of 2-3 years of nursing experience required; at least 1 year of experience in home health or community health preferred. Previous leadership/supervisory experience is a plus.
- Proficiency in using electronic health records (EHRs) and medical documentation systems
- Valid driver's license and/or access to a reliable vehicle or transportation for State travel based on geographical location of clients
Additional Information
- $40.00- $50.00 an hour + $2,000 Sign On Bonus!
