Registered Nurse (RN) – Home Health

Schedule: Full-Time, Mon through Fri

About the Role

As a Home Health Registered Nurse, you’ll deliver skilled nursing care to patients with acute, chronic, and/or terminal conditions who meet home health admission criteria. You’ll provide care in patients’ homes or facility settings, manage a caseload, complete admissions and assessments, and deliver ongoing skilled visits in compliance with regulatory requirements.

You’ll serve as a key coordinator of care, collaborating closely with physicians, the interdisciplinary team, patients, families, and caregivers to ensure each patient’s plan of care is comprehensive, effective, and compassionate.

What You’ll Do

• Deliver skilled nursing care aligned with Elara Caring’s mission, values, and clinical standards

• Perform admissions, initial assessments, and ongoing reassessments during each visit

• Maintain primary responsibility for assigned caseload, including planning, implementing, coordinating, and evaluating patient care

• Identify and address patient, family, and caregiver needs—physical, psychosocial, environmental, and spiritual

• Develop, implement, and revise individualized plans of care in collaboration with the interdisciplinary team and attending physician

• Communicate significant changes in patient condition or environment to physicians, Clinical Supervisors, and care team members

• Ensure accurate, timely documentation in accordance with regulatory, payer, and organizational requirements

• Maintain patient and staff privacy and confidentiality in compliance with HIPAA

What We’re Looking For

• Current, unrestricted RN license in the state

• Minimum of 1 year of nursing experience in a clinical setting

• Ability and willingness to travel within the branch coverage area (approximately 50%)

• Valid driver’s license, dependable vehicle, and current auto insurance

• Associate or Bachelor’s Degree in Nursing (preferred)

• Previous Home Health or Hospice experience (preferred)

• Experience with HomeCare HomeBase (preferred)
