About the RoleAs a Home Health Registered Nurse, you’ll deliver skilled nursing care to patients with acute, chronic, and/or terminal conditions who meet home health admission criteria. You’ll provide care in patients’ homes or facility settings, manage a caseload, complete admissions and assessments, and deliver ongoing skilled visits in compliance with regulatory requirements.
You’ll serve as a key coordinator of care, collaborating closely with physicians, the interdisciplinary team, patients, families, and caregivers to ensure each patient’s plan of care is comprehensive, effective, and compassionate.

Deliver skilled nursing care aligned with Elara Caring’s mission, values, and clinical standards
Perform admissions, initial assessments, and ongoing reassessments during each visit
Maintain primary responsibility for assigned caseload, including planning, implementing, coordinating, and evaluating patient care
Identify and address patient, family, and caregiver needs—physical, psychosocial, environmental, and spiritual
Develop, implement, and revise individualized plans of care in collaboration with the interdisciplinary team and attending physician
Communicate significant changes in patient condition or environment to physicians, Clinical Supervisors, and care team members
Ensure accurate, timely documentation in accordance with regulatory, payer, and organizational requirements
Maintain patient and staff privacy and confidentiality in compliance with HIPAA
Current, unrestricted RN license in the state
Minimum of 1 year of nursing experience in a clinical setting
Ability and willingness to travel within the branch coverage area (approximately 50%)
Valid driver’s license, dependable vehicle, and current auto insurance
Associate or Bachelor’s Degree in Nursing (preferred)
Previous Home Health or Hospice experience (preferred)
Experience with HomeCare HomeBase (preferred)
