Licensed Practical Nurse (LPN) – Skilled Home HealthFull-Time | Elkhart & LaGrange Counties

As a Skilled Home Health LPN, you’ll provide high-quality nursing care to patients in their homes under the direction of an RN or physician. You’ll play a key role in helping patients stay safe, comfortable, and out of the hospital.
Deliver skilled nursing care according to the plan of care and physician orders
Support patients and caregivers with education and self-care techniques
Observe, assess, and communicate changes in patient condition to the RN Case Manager
Document care accurately and timely in the EMR
Promote patient safety and help prevent avoidable hospitalizations
Build trusting relationships with patients, families, and care team members
Graduate of an accredited nursing program
Active LPN license in the state
At least 1 year of clinical experience (home health or hospice experience preferred)
Comfortable working independently in the field
Valid driver’s license, reliable transportation, and ability to travel within Elkhart & LaGrange Counties
Ability to meet physical demands of the role
