RN Case Manager

StartDate: 2/2/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization in an inpatient rehab setting (Adult and Geriatric units). Some local travel to a neighboring hospital may occur.

MINIMUM REQUIRED QUALIFICATIONS – CA RN license; BLS; 1-3 years acute hospital case management experience; Clinical background; Knowledge of post-acute care, care coordination, case management, business development/marketing.

PREFERRED QUALIFICATIONS – Understanding Of Insurance Authorizations And Process

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Monday through Friday with 1 weekend a month. The weekend shift is 4 hours Sat and 4 hours Sunday (they then flex off the following Thursday).
