Position Overview

The Registered Nurse will support facility-based care coordination activities within an interdisciplinary care delivery model. This role assists providers and clinical teams in managing member care, ensuring accurate documentation, coordinating services, and responding to changes in patient condition. The RN will work closely with Advanced Practice Clinicians and facility leadership to promote quality outcomes, education, and continuity of care.

Primary Responsibilities

• Ensure accurate and complete documentation in the medical record
• Collaborate and communicate with interdisciplinary care teams
• Assist providers and teams with care coordination activities
• Support benefit determination for Medicare Part A, Part B, and Part D
• Partner with the primary APC and facility staff to identify and respond to changes in patient condition
• Assist with therapy coordination and follow-up
• Participate in facility-based meetings
• Schedule and join family conferences, team meetings, and case presentations
• Coordinate training, tracking, and compliance with quality measures
• Assist in developing and delivering in-services for select groups
• Support providers with documentation accuracy, coding, medication reconciliation, assessments, and patient records
• Write verbal orders from providers in accordance with state RN practice laws and facility policies
• Assist with managing members through acute episodes or chronic condition exacerbations
• Support members during short term sub-acute rehabilitation episodes

Required Qualifications

• Active and unrestricted RN license in the state of residence or ability to obtain by start date
• Certified in Basic Life Support (BLS)
• Access to reliable transportation

Preferred Qualifications

• One or more years of clinical experience, preferably in a long-term care setting
• Experience with patients who have complex behavioral, social, or functional needs
• Understanding of geriatrics and chronic illness management
