Join Our Team as an RN Case Manager!

Position Details:

  • Title: RN Case Manager
  • Location: Boston, MA and surrounding towns.
  • Hours: Monday – Friday, Day Shift (32 or 40 hours weekly; options for job sharing 24/16 or 20/20 hours)
  • Pay Range: $45 - $60 per hour (dependent on experience and qualifications)
  • Benefits: None offered at this time
  • Pay Type: Per Hour

Key Responsibilities:

  • Conduct comprehensive assessments of hospice patients in home, skilled nursing, and inpatient settings.
  • Develop and update individualized plans of care in collaboration with the interdisciplinary team.
  • Provide expert clinical supervision to hospice aides and volunteers.
  • Administer specialized procedures such as venous access care, medication management, and infusion therapies (if qualified).
  • Support families through the caregiving process and bereavement care initiatives.
  • Maintain detailed, accurate documentation following organizational standards.
  • Participate in 24/7 on-call rotations, including holidays, ensuring continuous patient support.
  • Collaborate effectively with physicians, staff, and external agencies to optimize patient outcomes.

Required Skills & Qualifications:

  • Valid Massachusetts Registered Nurse license in good standing.
  • Associate’s degree in Nursing (Bachelor’s preferred).
  • Minimum of 3 years of nursing experience, including at least one in an acute or hospice setting.
  • Strong assessment, critical thinking, and communication skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Valid driver’s license and insured vehicle in good working order.
