RN Case Manager

StartDate: 3/23/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization

MINIMUM REQUIRED QUALIFICATIONS – CO/compact RN license; BLS; 1+ year recent case management experience in an acute hospital setting

LENGTH OF ASSIGNMENT – 4 weeks

SHIFT / HOURS PER WEEK – 8:00 AM-4:30 PM. Weekend Hours: 1-5.
