Job Summary
The RN Patient Care Coordinator is a member of the Nursing Dept. The RN Patient Care Coordinator has responsibility and accountability for the provision of nursing care. The RN Patient Care Coordinator manages care for patients and their families through the application of the nursing process utilization, independent judgment, collaboration and communication with all team members including ancillary and support services. The RN Patient Care Coordinator assists nursing staff members to develop a plan of nursing care and utilizes established clinical pathways which are implemented by the care delivery team under the supervision of the RN Patient Care Coordinator. The RN Patient Care Coordinator delegates patient care activities based upon patient needs and team skills/abilities and supervises the delivery of care by team. The RN Patient Care Coordinator maintains their clinical nursing skills and is the clinical expert at the bedside. The RN Patient Care Coordinator monitors the clinical skills and care delivery of the clinical nursing team and reports any issues to the Director/ Team Leader. The RN Patient Care Coordinator assists with the education of nurses on assigned unit under the supervision of the Director/ Team Leader. The RN Patient Care Coordinator evaluates patient response to treatment and nursing interventions and revises the plan of care as needed. The RN Patient Care Coordinator monitors patient progress and prepares patient for discharge in collaboration with the clinical team members.
Requirements
Associate's degree required, Bachelor's preferred. Encourage Medical-Surgical or Orthopedic Certification or working towards an advanced agreed. Maryland Registered Nurse License, American Heart Association CPR Healthcare Provider Certification.
