Job Summary And Qualifications

The VCP Case Manager will play a critical role in developing and leading the execution of the patient population case management strategy. Responsible for providing coordination of care for patients to support safe, seamless, timely transitions across the continuum. Utilizing a collaborative process, the case manager will identify, assess, plan, implement and evaluate the options and services required to meet an individual’s health and health related needs, including social- determinants that affect ones’ overall wellbeing. Services resulting in improved patient experience and reduced cost outcomes for the overall patient attributed population.

  • Position is primarily remote work but candidate must live within the Richmond, VA market.
  • Identify attributed patients who benefit from care coordination and work with practices to effectively manage the high risk, high cost, and high utilizers.
  • Develop and maintain community and payer resource directories.
  • Assist with referring to appropriate ancillary services such as CM/DM programs, BH resources, pharmacist support, and other resources as identified.
  • Assist and perform transitions of care and complex care management for the identified population.
  • Outcomes to include reduced emergency visits and inpatient admissions.
  • Provide patients appropriate resources to help overcome barriers to care.
  • Identify and address issues, determine reasons and take action to resolve, and escalates appropriately; working with the quality manager as needed for patients with avoidable emergency room and inpatient stays and high utilization.
  • Communicates clearly, regularly and effectively with all levels and functions to create an atmosphere of mutual respect, openness and encouragement.
  • Maintains the confidentiality of the meetings attended.
  • Builds rapport and establishes credibility with the patients, practices and providers.

What qualifications you will need:

  • Registered Nurse
  • Case management certification preferred
  • Previous case management and utilization review experience preferred
