Position Description
Function:
Responsible for the health-related components of the River House Adult Day Center Program.

Responsible for overseeing health-related components of the River House Adult Day Center program.

Key Responsibilities
Health Program
- Develop, implement, and ensure compliance with nursing policies, procedures, and regulatory standards.
- Supervise, train, and evaluate one part time nurse and four certified aides.
- Provide direct clinical care, including assessments, medication administration and monitoring, minor wound care, diabetic care, and mental status observation.
- Develop, review and update individualized Care Plans at intake and at least every six months.
- Coordinate services outlined in Care Plans, including physician orders, therapies, nutrition plans, personal care, and rehabilitation services.
- Maintain current knowledge of medical, social, and regulatory issues impacting the River House population.
- Prepare and submit reports to outside agencies as required.
Family & Caregiver Support
- Collaborate with families and caregivers on health-related matters.
- Provide recommendations, referrals, and care coordination support.
- Incorporate family input into Care Plan reviews and updates.
Administration & Personnel
- Oversee administrative practices related to the Health Program, including staff orientation, supervision, evaluations, and in-service training.
- Assist with preparation of the annual health budget and maintain supply inventories.
- Ensure employee health requirements, including physicals and PPDs, are current.
- Assume administrative responsibility in the absence of the Executive Director.
Other Duties
- Maintain a safe, therapeutic, orderly, and secure environment.
- Provide high-quality customer care to clients and families.
- Participate in required nursing education and training.
- Perform special projects and other duties as assigned.
- Lead emergency response procedures for clients and staff.

- Maintain strict confidentiality of client, caregiver, and organizational information.
Qualifications
- Registered Nurse with a minimum of two years of experience in gerontology or community nursing.
- Current licensure in the State of Connecticut.
- Strong organizational, leadership, and record-keeping skills.
- Ability to communicate effectively with staff, participants, and caregivers.
- Compassionate and client-centered approach to care.
Salary and Benefits
- Start date: April 2026
- Salary commensurate with experience
- Paid Time Off: 21 days
- Paid Holidays: 9 days including the day after thanksgiving
- 401(k) eligibility after one year of employment

Schedule and Work Location
- Monday–Thursday, 8:00 a.m.–4:00 p.m. (onsite)
