A healthcare center in Bronx is seeking a compassionate and reliable Licensed Practical Nurse (LPN) to join its Long-Term Care / Rehab team. This is a great opportunity for nurses who are passionate about providing high-quality care to residents in a supportive and collaborative environment.

• Position: Licensed Practical Nurse (LPN)

• Specialty: Long-Term Care / Rehab

• Location: Bronx, NY

• Shift: 4:00PM - 12:30AM and Midnight - 8:30AM

• Schedule: Full-time or part-time available

• Weekend Rotation: Required

• Pay Rate: $42 - $45 per hour

• Provide direct patient care following the nursing process and established care plans

• Support residents in maintaining optimal health, comfort, and well-being

• Administer medications and treatments according to physician orders and nursing standards

• Observe, document, and report changes in resident condition to the Registered Nurse

• Maintain resident privacy and confidentiality

• Collaborate with residents, families, physicians, and interdisciplinary care teams

• Promote a safe, clean, and supportive environment for residents and staff

• Assist with orientation and mentoring of new staff

• Ensure compliance with ethical and legal nursing standards

• Complete all shift duties and assist with additional responsibilities as needed

Education

• Graduate of a National League of Nursing (NLN) accredited practical nursing program

Licensure

• Current New York State Licensed Practical Nurse (LPN) license

Experience

• Previous experience in long-term care, rehabilitation, or a similar nursing setting preferred
