Summary:  Healthcare professionals positively impact the lives of people living with chronic illness and disability by addressing a broad range of issues related to well-being, including assistance in navigating the healthcare system and finding available community resources. We often remain with our clients through the end of their lives. Sometimes we may not be comfortable with discussing and providing end of life care and may benefit from specialized knowledge and skill. This presentation will provide you with best practice in the treatment of serious illness, especially in the last 6 months of life, based on experiences of a hospice worker and an end-of-life doula. A case study will demonstrate how collaborative community partnerships deliver quality goal-concordant care until the last breath.
